Decide whether it will be used for official or personal use, whether you need to embed logos in it, whether the printable form will need to filter any data from the company software etc. Once you have identified the function the printable form is expected to perform, you can easily decide which software or online program you need to create the form.
Once employed, the employee usually fills out an employee form that he needs to fill out with the information requested. An employee form template would normally look like a form with the name of the company on top, and a title stating New Employee Form. An employee form template would normally ask for the name of the person, his department or his position title.
